What data do we collect?

Our Company collects the following data (“Personal Data”, “information”, “data”, ”Personally Identifiable Information”, “PII”):

Personal identification information such as:

  • The name of your pharmacy and/or your pharmacy account number

  • The name of your care provider and your medical ID number you use with your care provider

  • The name of your medical insurance company and your medical insurance ID number.

  • The name, email and phone number of the person attending to you and accessing the Sites on your behalf.

  • Any medications you are taking or may take, the quantity and schedule of dosage (your Prescriptions)

How do we collect your data?

Directly Collected Data

You directly provide Our Company with most of the data we collect. We collect data and process data when you use our Sites or Service to:

  • Register online or complete a user profile form.

  • Complete a form or questionnaire related to a trial or information we require.

  • To perform scheduling for reminders or appointments

  • A customer survey or provide feedback on any of our Sites on social media chat or message boards

When we receive your data indirectly from the above sources we will be acting as the “Data Processor”, which means we will process your data on behalf of the entity (the “Data Controller”) who supplied us or gave us permission to access your data.

How will we use your data?

Our Company collects your data so that we can:

  • Supply you and other interested medical parties (e.g. your Care Provider) information, reports and progress related to your Prescriptions, trials and other medical information supplied by our Service.

  • Improve our products and services.

  • Use anonymized data for future clinical analysis, to establish industry baselines, identify trends.

We do not sell your Personally Identifiable Data (PII) to our partners or other third parties.We may use your personal information for automated individual decision-making or profiling as permitted by law to monitor and prevent fraud and abuse on our systems, to ensure the security and reliability of our services, and as needed to enter into or perform our contract with you. We will not use any special categories of personal information for these purposes.

How do we store your data?

  • Our Company securely stores your data at Sites that are hosted in the United States. The Sites are subject to U.S. state and federal law. If you are accessing our Sites from other jurisdictions, please be advised that you are transferring your personal information to us in the United States, and by using our Sites, you are agreeing to that transfer and use of your personal information in accordance with this Privacy Notice. How long do we keep your data?

  • Our Company will keep your Personal Data as long as you use the Service or as required by our partners whom we are contractually obligated to process your data on their behalf. We also may keep your data to comply with regional regulatory and legal requirements. If you no longer use our Service or you request the right of erasure or we no longer need your data to complete required processing and your data is no longer needed to satisfy contractual, regulatory or legal requirements, we will permanently delete your data from our Sites.

  • Please Note: We may keep anonymized records or data of clinical trials or other transactions you performed using our Service. We keep this data for future clinical analysis, to establish industry baselines, identify trends and to improve our products and services. These anonymized records contain no Personally Identifiable Data (PII) that can be associated back to you.

  • Our Company may send you information about products and services of ours that we think you might like, as well as those of our partner companies.
